There is no cross reactivity with N- or C-terminal fragments. Osteocalcin, the major non-collagenous protein of bone, is a specific product of the osteoblast and its synthesis is stimulated by 1,25-Dihydroxy vitamin D3 . A fraction of that synthesized does not accumulate in bone but is secreted directly into the circulation. Animal studies indicate serum osteocalcin is derived from new cellular synthesis rather than from the release of bone matrix protein during resorption. Furthermore, serum osteocalcin concentrations are related to histomorphometric measurements of bone formation but not resorption. Deviations from normal concentrations of circulating osteocalcin are a consequence of changes in the synthesis or degradation of the protein. Such changes may result from or may be associated with specific disease states.
